NPLC’s are a new model for delivery of primary health care in Ontario. The model is designed to improve access to care for individuals and families who do not have a primary healthcare provider(i.e. Family doctor or Nurse Practitioner). One of the unique aspects of the model is nursing leadership within an interprofessional team. NPLC’s bring the focus of wellbeing, health promotion and disease prevention, to the day to day delivery of care.
